Part 2 — jawline series
Our jawline filler article explained how we add volume along the jaw to sharpen it. This one is the exact opposite: how we reduce muscle bulk so the jaw looks slimmer. Two different problems, and the tool for each is the reverse of the other.
Two very different types of patient come in for this treatment:
The first says: “My face looks square along the bottom and I’d like it softer.” The second says: “I wake up with jaw pain, and my dentist told me I grind my teeth at night.”
What’s striking is that it’s the same treatment. Botox to the chewing muscle solves both, because both share one cause: a muscle working harder than it needs to.
The short answer
Masseter Botox is injected into the chewing muscle on both sides of the jaw. As the muscle relaxes, its strength decreases and its bulk gradually reduces. The result: the jaw angle slims, giving the face a longer shape, and teeth grinding and jaw pain ease. Pain relief shows within one to two weeks; the change in jaw shape needs 6 to 8 weeks.
What is the masseter?
Put your hand on the side of your face at the jaw angle and bite down firmly. The part that bulges under your hand is the chewing muscle.
It’s one of the strongest muscles in the body relative to its size. And like any muscle, the more it’s used the larger it grows. Anyone who grinds at night, chews a lot of gum, or carries chronic tension will develop a bulkier muscle over time.
The result: a wider-looking jaw angle, and a face that looks square along the lower third.
How is it injected?
Injection goes into specific points within the body of the muscle on both sides. This area demands high anatomical precision, because neighbouring muscles affected in error can cause problems with the smile.
The session itself is quick — 10 to 15 minutes. Discomfort is minimal; most patients describe a light prickling.
An important point: the dose here is markedly higher than for other facial areas, because the muscle is large and strong. So if someone offers you “masseter Botox” at the same price as forehead Botox, ask how many units.
The results timeline
| Timing | What’s happening | What you’ll notice |
|---|---|---|
| First 3 – 7 days | Muscle begins relaxing | A sense of lightness, less clenching |
| Week 1 – 2 | Clear relaxation | Pain and morning headaches improve |
| Week 4 – 6 | Muscle atrophy begins from reduced use | Jaw starts looking slimmer |
| Week 6 – 8 | Clear atrophy | Best result in jaw shape |
| Month 4 – 6 | Gradual return of activity | Time for the next session |
Note the key difference: pain relief is fast, shape change is slow. If you’re coming for the aesthetic result, be prepared to wait two months.
Masseter Botox or jawline filler?
This is the most commonly confused point, and the two are exact opposites:
| Masseter Botox | Jawline filler | |
|---|---|---|
| What it does | Reduces muscle bulk | Adds volume over bone |
| Result | Slimmer jaw, longer face | Sharper, more defined jaw |
| Suits | Wide jaw from muscle bulk | Undefined or weak jawline |
| Results appear | 6 – 8 weeks | Immediately |
| Lasts | 4 – 6 months | 9 – 18 months |
So if your jaw is wide from muscle, Botox. If your jaw lacks definition, filler. And in some cases both are done — Botox to reduce width, filler to define the front line.
For detail on the other option, see our article on jawline definition with filler.
Dr. Rehab Zakaria’s advice: “The first thing I do with anyone coming for masseter Botox is place my hand on the muscle and ask them to clench. If the muscle bulges clearly, Botox will make a difference. If the width comes from the shape of the bone rather than the muscle, Botox will do almost nothing — and that has to be said before anyone pays.”
The medical side: teeth grinding
This isn’t a purely cosmetic use. Bruxism causes genuine problems:
Enamel wear. Recurrent morning headaches. Jaw joint pain and clicking. Neck and shoulder tension. Disturbed sleep.
Masseter Botox reduces the force of contraction, which noticeably eases these symptoms for many patients.
But let’s be precise: it treats the symptom, not the cause. If the underlying driver is psychological stress or a bite problem, that needs addressing separately. Which is why coordination with your dentist is part of the plan in these cases, not an optional extra.
Who isn’t a candidate?
We decline or postpone in: pregnancy and breastfeeding. Neuromuscular conditions such as myasthenia gravis. Known allergy to product components. Infection at the injection site. Certain medications that interact with the toxin, without medical coordination.
Also: if your jaw width comes from bone structure rather than muscle, this treatment won’t help you. Assessment beforehand is what distinguishes the two.
Expected side effects
In the first week or two you may notice: mild muscle fatigue when chewing something hard like nuts. An unfamiliar sensation while chewing. Occasionally slight, temporary asymmetry if the two sides respond differently.
These all settle as you adapt.
What is not expected: genuine difficulty chewing or speaking, or a visible change in your smile. If that happens, contact your doctor — it’s usually related to excessive dosing or imprecise placement.
Frequently asked questions
Will my face widen again if I stop?
Yes, the muscle gradually returns to its previous size if you stop. But with regular repetition, many patients find the interval between sessions lengthens.
Does it slim the whole face?
No. It works on the lower jaw angle only. Cheeks and facial fat are unrelated to this muscle.
Is it suitable for men?
Yes, and it’s common — particularly for grinding. The dose is usually higher because the muscle is larger.
How often is it repeated?
Every 4 to 6 months initially, with intervals often lengthening over time.
Will it change my smile?
With precise injection, no. Those problems occur when product reaches neighbouring muscles, which anatomical experience prevents.
The bottom line
Masseter Botox serves two purposes at once: slimming the jaw angle where the width comes from muscle bulk, and easing teeth grinding and the jaw pain that goes with it.
The medical result appears quickly. The aesthetic result needs two months of patience. Both depend on a careful assessment establishing whether the muscle is genuinely the problem.
At Dr. Rehab Zakaria’s clinic in El Tagamoa, New Cairo, that examination happens by hand before any injection — because the answer determines whether the treatment is appropriate at all.
